Terms & Conditions

  • This form is for WUSA-affiliated student groups (i.e. Clubs, Services, Societies, etc.) only
  • All requests are subject to ITMS approval, at discretion of ITMS management
  • Equipment loan requests require at least three business days’ notice
  • Damage to equipment or returning the equipment late will result in additional fees.
  • Equipment is for short term use only
  • Visual Events Services (i.e. projectionists) requests require two weeks’ notice
  • For on-campus use only

As the primary contact, I agree to be personally responsible for ITMS equipment from pick up to drop off and/or in coordination with ITMS personnel. ITMS equipment must be picked up and returned by one of the listed contacts (primary or secondary) to the ITMS Office (MC 1088) in the same working condition as loaned.
